The Warrington health and social care system are pleased to announce that the new Rapid Community Response Service (RCRS) is in operation.
Not yet fully staffed to full capacity, the service is currently focusing on referrals from:
The RCRS provide urgent (in two hours) assessment and support (in two days) for a short time, to help people recover and retain independence. The team is particularly important in during the current coronavirus pandemic as they may help people:
The RCRS team is made up of nurses, social workers, therapists and care staff who can triage, assess and support people during a crisis. People eligible for the service are:
We expect the team will likely focus on people aged over 65, who have an exacerbation to crisis point of a physical long-term condition(s).
As we are one of seven national 'accelerator sites', and therefore part of a national development programme, we would welcome feedback on the service in terms of what is working well and how it can be improved.
